Question:

An applicant is considering changing the site/location of SHP project based assistance. Should this change be reflected in Exhibit 2?

Answer:

No. Applications for renewal funding do not have to include the specific address of each site/structure. Site addresses are reported to the local HUD CPD office before grant agreement. Any change to the location of an existing site is considered a significant change requiring approval from the local HUD CPD office before execution of the renewal grant agreement.

For applications for new funding, form 4C (Project Location(s)) of the Exhibit 2 application should include the address of each site/structure proposed to be used for housing or providing supportive services to project participants. However, the site locations are not required at the time of application. The one exception is new S+C SRA projects; the address of each SRA structure is required at the time of application. For all other new applications, the actual address of each site must be reported to the local HUD CPD office BEFORE execution of the new grant agreement.
