Products related to developing, financing, and implementing long-term plans of action designed to end chronic homelessness or prevent high-risk persons from becoming chronically homeless. Strategies for ending chronic homelessness often focus on street and shelter outreach, effective engagement and assessment techniques, provision of immediate shelter (safe havens and/or housing first strategies), and intensive supportive services. Prevention strategies often focus on people who are leaving hospitals, psychiatric facilities, substance abuse treatment programs, correctional institutions, and foster care.
